   This order form is for a Kidde Supra Max residential lock box with door mount hander.  Once ordered, a member of the Fire Marshal's Office will contact you to schedule an installation date.  

   Please make sure that you are available for the appointment and to have a check available for the installer for the full amount and made payable to WFFPAC.

   If you need to reschedule or cancel, please contact the Fire Marshal's Office at least one business day before your scheduled appointment.
